Case Manager – Chronic Disease Intermediate Care Service

Joining a strong multidisciplinary team offering client focused services, you will utilise a chronic disease self-management framework to support clients to optimally manage their health and wellbeing, and reduce hospital admissions. With a focus on Respiratory, Cardiac and Diabetes services, the Allied Health Chronic Disease Intermediate Care Service brings together the fields of Dietetics, Exercise Physiology, Occupational Therapy, Podiatry, Physiotherapy, Speech Pathology, Social Work and Case Management. In this role, you will apply your clinical experience, increasing clinical knowledge and professional competence to plan, implement and evaluate comprehensive and integrated services to meet the needs of clients. This will be achieved by working collaboratively with services from the Northern Adelaide Local Health Network Clinical Divisions and Aboriginal Health Services to optimise health outcomes and reduce hospital admissions. Working under limited direct clinical supervision, you may provide direction and support to less experienced therapists, allied health assistants and students.
Holding a degree or equivalent qualification in a relevant Allied Health profession, you will have demonstrated experience working in a case management role within a multidisciplinary service team. This will include clinical experience in broad range of conditions, particularly Chronic Diseases including Diabetes, Cardiac and Respiratory Diseases. You will possess sound competency and knowledge in case management, and Chronic Disease Self-Management Support, including assessment techniques and evidence based interventions. A confident communicator who can work well as part of a multidisciplinary team, your strong interpersonal skills will equip you to interact effectively with people from a diverse range of backgrounds.

 


Occupational Therapist/Case Manager – Chronic Disease

You will provide assessment and treatment services within a Chronic Disease Self-Management framework for patients of the Northern Adelaide Local Health Network (NALHN) Chronic Disease Intermediate Care Service. The Intermediate Care Services bring together Dietetic, Exercise Physiology, Occupational Therapy, Podiatry, Physiotherapy, Speech Pathology, Social Work and Case Management services working collaboratively with services from the NALHN Clinical Divisions and Aboriginal Health Services to optimise health outcomes and reduce hospital admissions for patients with Diabetes, Cardiac and Respiratory Diseases, and vulnerable people with other chronic diseases. You will plan, implement and evaluate comprehensive and integrated services to meet the needs of clients, and liaise directly with medical and nursing staff and other community agencies. Working under limited direct clinical supervision, you may be required to provide direction and support to less experienced therapists, allied health assistants and students.

For appointment as an Occupational Therapist, you will need to hold a degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia. You will have clinical experience in broad range of conditions, particularly Chronic Diseases including Diabetes, Cardiac and Respiratory Diseases. With demonstrated competency and knowledge in case management and Chronic Disease Self-Management Support, you will be skilled with assessment techniques and evidence based interventions. A confident communicator who can work effectively in an inter-disciplinary team setting, you will possess the time management skills required to successfully manage a caseload.
